PRACTICE POLICIES

By accessing or interacting with our website or booking an appointment with us, you agree that you have read and understand the following policies. You also agree to abide by all of them and understand that violating any of these policies may result in termination of your relationship with Archway Aesthetics and denial of future appointments at the sole and exclusive discretion of management. You also agree that you have had ample time to review these policies and have had any and all of your questions about them answered.
CANCELATION / NO SHOW
- We respectfully request at least 24 hours’ notice for cancellations.
- No-shows or notification given less than 24 hours prior to your appointment time will result in a cancellation fee of $100.
- A credit card will be required to reserve an appointment. You will not be charged for your service until the time of treatment. If you cancel or do not show up for your appointment a fee of $100 will be applied to your card.
- Guests who no-show or give notice less than 24 hours prior to their appointment on two separate occasions will have their relationship with Archway Aesthetics terminated and be denied future appointments at the sole and exclusive discretion of management.
Cashless operation
- Archway Aesthetics is a cashless operation. We only accept payment via credit card or financing through our financing partners.
- We do not accept or take insurance of any kind.
EARLY CLOSURE / INCLEMENT WEATHER
- In the event of inclement weather, we will reschedule Guests the night before their scheduled appointment. If the practice needs to close early due to inclement weather, we will contact all affected Guests and give them priority booking to reschedule. Notification of early closure will be posted on social media, front door, and with the message service.
financial responsibility
- YOU MUST PROVIDE YOUR PICTURE IDENTIFICATION TO THE RECEPTIONIST FOR PHOTOCOPYING AT EACH APPOINTMENT. THIS ACCOUNT IS SELF-PAY AND PAYMENT IN FULL IS DUE AT THE TIME OF EACH SERVICE.

- Clearly understand and agree that all services rendered to me may be charged directly to me and that I am personally responsible for full payment. I understand that even if I suspend or terminate treatment, any fees for professional services rendered to me or to my dependent up to the point of termination will be immediately due and payable.
- Acknowledge that I am responsible for any outstanding fees for services provided to me by Archway Aesthetics (“Practice”).
- Understand that any other arrangements that may involve payment plan or payment deferral, must be made in writing with the office manager or business manager of the Practice. Verbal agreements are not acceptable.
- Acknowledge that the Practice reserves the right to charge a fee if I do not attend or cancel the scheduled appointment without providing 24-hour prior notice to the Practice. I further acknowledge the Practice reserves the right to reschedule my appointment if I am more than 15 minutes late to the scheduled appointment.
GIFT CERTIFICATES
- Gift Certificates are not redeemable for cash and are non-refundable. Gift Certificates and Gift Cards must be present at the time of service.
MEMBERSHIPS
- Memberships are non-transferable and non-refundable. Memberships may be terminated at any time by a Member provided that at least 30 days’ notice is provided in advance of your monthly dues due date.
- Please refer to the membership agreement for the cancellation process and additional terms and conditions.
minors
- We love children, but for the safety and comfort of all of our Guests, we kindly ask that you do not bring children to your appointment.
- You must be at least 18 years old to receive treatment at Archway Aesthetics.
pets
- We love pets, but for the safety and comfort of all of our Guests, we kindly ask that you do not bring pets to your appointment. Service animals as defined by the Americans with Disabilities Act of 1990 are excluded from this policy.
Rescheduling Guests due to Provider Absence
- In the event that a Provider calls out and is unable to fulfill their shift, another provider will be contacted to take the shift. All Guests will be called to inform them that their Provider is unable to service them and offer another day or with the substitute Provider.
RETURNED PRODUCT
- Product returns or exchanges are accepted up to 7 days after purchase with a receipt. If the product is unused and in the original packaging we will refund the purchase amount to the original form of payment. If the product has been used we can offer an exchange or credit for the value of the product.
RUNNING LATE
- Please call us if you are running late. Appointments more than 15 minutes late are subject to rescheduling, per Provider and management discretion.
ZERO-TOLERANCE POLICY
- Archway Aesthetics has a zero-tolerance policy when it comes to carrying, consuming, or arriving to an appointment while under the influence of alcohol or any illegal substance on Company property. Additionally, we have a commitment to providing a safe and dignified environment for all Guests and Team Members, regardless of gender, race, ethnicity, sexual orientation, disability, religion, or any other aspect of their identity. As part of that commitment, Archway Aesthetics has a zero-tolerance policy when it comes to harassment, verbal abuse, bullying, discriminatory behavior, micro-aggressions, or any other behavior that might make a Guest or Team Member feel uncomfortable. Management reserves the right to dismiss any and all Guests who violate this policy at its sole and exclusive discretion.
